Documentation Policy
Important information about our limits regarding court-ordered treatment, documentation, and administrative requests.
Court-Ordered Treatment & Documentation
We do not provide services or documentation for court-ordered matters, including but not limited to:
- Domestic violence cases
- Mandated treatment/therapy
- Custody evaluations or arrangements
- Legal case
Emotional Support Animals (ESA)
We also do not complete paperwork for Emotional Support Animals (ESA).
FMLA & Disability Documentation
If you are seeking documentation for the Family and Medical Leave Act (FMLA) or disability-related accommodation:
- You must attend several sessions before any paperwork can/will be considered.
- Completion of documentation is at the therapist’s discretion.
- A documentation fee of $50 (due before paperwork is provided) applies to FMLA or ADA accommodation forms (not billable to insurance); additional fees may apply for other requests.
Processing & Confidentiality
- All documentation is completed in alignment with confidentiality guidelines.
- A signed release of information form is required to share documentation with any third party.
- Please allow up to 14 business days to complete documentation requests (ESA, FMLA, Accommodations).
